The phrase “laser facelift” sounds like a contradiction — a facelift without surgery? But the Fotona 4D has earned its reputation by doing something genuinely clever: it works on your face from the inside and the outside at the same time, with no needles, no scalpel, and no real downtime. Here’s the plain-language guide to what it is, how it works, and what the research actually supports.
What the Fotona 4D is
Fotona 4D isn’t a single zap — it’s a sequence of four treatment modes delivered in one session, using two different laser wavelengths working together:
- An Er:YAG laser (erbium), which is excellent for the surface and shallow layers of the skin.
- An Nd:YAG laser (neodymium), which penetrates deeper to heat tissue and stimulate collagen.
The “4D” refers to treating the face across multiple planes and depths — including, uniquely, from inside the mouth.
How it works, step by step
The four modes each do a different job:
- SmoothLiftin (intraoral) — This is the signature move. The laser is applied inside the mouth, gently heating the tissue around the nasolabial folds and mouth area from behind. This tightens the skin where folds form, without touching the outer surface.
- FRAC3 — A deeper, fractional Nd:YAG pass that targets imperfections and stimulates collagen below the surface.
- PIANO — A bulk-heating mode that warms the deeper tissue across larger zones for an overall tightening and lifting effect.
- SupErficial — A light Er:YAG peel that polishes the surface, refining tone and texture and adding glow.
Together, these heat the skin’s collagen at multiple depths, which both contracts existing fibers (immediate tightening) and triggers new collagen production over the following weeks (gradual firming and lifting).
What the science says
The individual wavelengths behind Fotona 4D are well studied. A randomized study comparing long-pulse Nd:YAG with fractional Er:YAG found both lasers effectively improved facial wrinkles, confirming the value of each tool the 4D combines.¹ Sequential photothermal resurfacing that pairs a variable-pulse Er:YAG with a scanner-assisted Nd:YAG — essentially the surface-plus-depth strategy at the heart of Fotona 4D — has been described for fractional resurfacing and remodeling.²
The intraoral SmoothLiftin step has its own supporting evidence. Is it safe? How long do results last?
Fotona 4D is non-ablative to minimally ablative depending on the mode, which means downtime is typically minimal — some redness and warmth that settle quickly.⁵ Because it relies on heat rather than injecting or cutting, it has a favorable safety profile in trained hands. Most patients do a series of 3–4 sessions spaced a few weeks apart, with results that continue to improve over 2–3 months as collagen rebuilds. Maintenance sessions help sustain the effect.
